Special Transport Permit - What information is needed before a Special Transport Permit can be used?

Information on the transport vehicle is necessary, i.e.  license number, vin number, make and year.  Information from the mobile home that is needed is the year, model, vin number, width and length.  Also where the home is coming from and going to is required along with the date and route to be taken.
