Marriage Application Request Form
This form must be signed, notarized and accompany payment when requesting a copy of a Marriage License Application. Copies of Marriage License Applications can only be requested by either party of the marriage or an immediate family memeber. Reference C.R.S. 24-72-204(3)(a)(XIX)(A)
Marriage License Request Form
This form must be filled out, signed and accompany payment when requesting a copy of a Marriage License. Copies of Marriage Licenses (without application) are public record and anyone may obtain a copy. This form does not require a notary.
