The US Department of Energy provided the Data Validation Package for the June 2011 Groundwater and Surface Water Sampling at the Durango, Colorado, Disposal Site and Processing Site, located at the Smelter Disposal Site. Marriage Application Request Form
This form must be signed, notarized and accompany payment when requesting a copy of a Marriage License Application. Copies of Marriage License Applications can only be requested by either party of the marriage or an immediate family memeber. Reference C.R.S. 24-72-204(3)(a)(XIX)(A)
Marriage License Request Form
This form must be filled out, signed and accompany payment when requesting a copy of a Marriage License. Copies of Marriage Licenses (without application) are public record and anyone may obtain a copy. This form does not require a notary.
