How to Use Fill-In Application Form:
You need to have Version 4.0 or higher of the free Adobe Reader  in order to use the "fill-in" feature of the on line application form.

• MUST be printed on legal size paper 8 1/2 x 14.  You must select File, Page Setup, change paper size from Letter to Legal, click OK.

  Use the "tab" key to move through the fields on a form.

  Use the mouse to select (click on) a specific field.

  In order to use the checkboxes on the on line application form, you will need to click the box with the mouse or use the space bar in the field.

• Print your completed application by using the print button on Adobe Reader, not the one on your browser.
